SEO for chiropractors: proximity decides the map pack, so the condition page has to do the rest

How a practice ranks when patients will only travel a few miles: profile categories, pages by condition and by technique, the booking widget problem, and review replies that stay inside HIPAA.

Chiropractic search is unusually lopsided. Because patients attend two or three times a week, they filter hard on distance, and the map results that sit above the ordinary listings are ranked heavily on how close you are to the person searching. That means a practice cannot out content its way into a neighbourhood four miles away, and the highest return work is always the same: a profile built out to the last field, reviews arriving steadily, and a set of pages that answer the condition and technique questions patients type before they are ready to book. The organic pages are where you win the searches the map cannot give you.

Set up the profile like the ranking factor it is

For a practice, the Google Business Profile does more than the website. Four fields matter more than the rest.

  • Primary category. Chiropractor, in nearly all cases. Secondary categories should reflect what you genuinely offer, such as sports medicine clinic, massage therapist or physical therapy clinic if those services exist in house with qualified staff. Adding a category you cannot deliver gets you calls you have to turn away.
  • Services. List them individually: chiropractic adjustment, spinal decompression, soft tissue work, prenatal care, sports injury assessment, custom orthotics. Each one is a thing people search.
  • Photographs. The building from the street so a first time patient recognises it, the parking, the waiting room, the adjusting table, the doctors by name. Stock images of a spine model help nobody.
  • Hours, including the early mornings and evenings. A practice that opens at seven wins the before work patient, and it only wins them if the hours are correct in the listing.

Keep the business name exactly as it appears on your signage and licence. Adding a condition or a city to the name field is the most common cause of a suspended listing in this category.

Two kinds of page carry chiropractic organic search

The first is the condition page, and it is the unit of work. One page per problem a patient would name: lower back pain, sciatica, neck pain, headaches and migraine, whiplash after a car accident, disc herniation, pregnancy related pain, scoliosis, sports injuries, work injuries. Each one written for a person who is currently in pain and is deciding whether this kind of care applies to them at all.

The second is the technique page, which almost no practice builds and which is specific to this field. Patients and other practitioners search by method: Gonstead, Activator, Thompson drop, Graston and instrument assisted soft tissue work, flexion distraction, non surgical spinal decompression, dry needling where your state permits it. A page naming the technique, who in the practice is trained in it and what a session feels like picks up search with almost no competition, and it is the page a patient sends to their spouse.

Both kinds of page carry the same furniture: the named doctor who provides the care with their credentials and licence, how many visits a case like this usually takes, what happens if the case is not appropriate for chiropractic and gets referred out, and a booking path above the fold. Health content is scrutinised harder than most, so authorship and credentials are not decoration here.

Keep your booking platform from eating your search presence

Practice management and booking systems are the most common technical problem in chiropractic SEO, and there are three distinct versions of it. The booking flow sits on a vendor subdomain or a vendor domain, so the page a patient actually converts on is not yours and builds nothing for you. The patient portal gets crawled and indexed, putting login and account pages into search results. And the new patient intake forms sit behind a widget that no crawler can see, so a page that looks like a booking page is, to a search engine, an empty page.

Fix it in this order. Make sure every condition and service page is on your own domain with its own indexable text, and treat the vendor's pages as the final step rather than the landing page. Block portal, account and form submission paths from indexing. Put a plain phone number and a visible appointment request on every page, so there is a path that does not depend on a script loading.

Reviews, which are both a ranking input and a HIPAA exposure

Review volume and recency move chiropractic rankings as much as anything on the site, and this is the one category where the obvious way to handle them is unlawful. A practice that submits claims electronically is a covered entity, and a public reply that confirms somebody was a patient discloses their care. So the mechanics matter:

  • Ask every patient at a natural moment, usually the visit where they tell you something has improved, and ask by text with a direct link.
  • Never incentivise positive reviews specifically and never filter who gets asked by how happy they seem. Both are practices United States regulators have moved against directly.
  • Reply to good reviews warmly without naming a condition or confirming treatment.
  • Reply to bad reviews with a thank you, a statement that you take feedback seriously, and a phone number, and nothing else. No defence, no detail, no hint that you know who they are.

Our guide to improving your Google rating covers the asking mechanics and the arithmetic of moving an average.

What chiropractic SEO costs and what to measure

A single location programme covering the profile, citations, condition and technique pages, technical work and review generation runs roughly $900 to $2,600 a month in my reading of this market, with a one off audit and citation cleanup of $400 to $1,100 at the start. Individual condition pages written properly, with a clinician reviewing them, run $180 to $450 each. None of that is a published rate card; it is my own working read.

Measure three things. New patient calls and form submissions, separated from existing patient traffic, which means tracking the appointment request and the phone separately from the portal login. Map pack position for your main terms measured from a point inside your actual radius rather than from the city centre, because a city wide average tells you nothing when your catchment is four miles. And which condition pages produce bookings, so you know which problem to write about next.

SEO questions from chiropractors and practice managers

How long does chiropractic SEO take to bring new patients?

Profile work and review flow start affecting the map results within weeks. Condition and technique pages typically build over three to six months as they accumulate signals. Plan on that order: the fast work first so there is something arriving while the pages mature.

Should a chiropractor write pages for conditions or for treatments?

Both, and keep them separate. Patients in pain search the condition. Patients who have already decided on chiropractic, and referring practitioners, search the technique. Link each condition page to the techniques you would use for it and each technique page back to the conditions it suits.

Why is my chiropractic office not showing in the map results?

Usually distance from where the search happened, an incomplete profile, or too few recent reviews. Check your position from several points inside your real catchment before concluding anything, because the result changes street by street in this category more than in almost any other.

How many reviews does a chiropractic practice need to rank locally?

Steady and recent beats a large total. A practice adding four or five reviews a month, all answered, outperforms one with a bigger number that stopped two years ago. The pattern of arrival is the signal.

Does blogging help a chiropractic website rank?

General wellness posts do very little. Pages that answer one specific question a patient in pain would type, written or reviewed by a named doctor in the practice, do a great deal. Write fewer pages and make each one the complete answer.

Can a chiropractor rank for a city they are not located in?

In the ordinary listings, yes, with a genuinely useful page about the care you provide and evidence of patients from that area. In the map results, distance limits it. Be honest with yourself about whether a patient from that city would really drive to you three times a week.

The assistant answers the pain question before the practice gets a chance

Condition pages exist because somebody types a question about their back and reads what comes back. That reading increasingly happens in a conversation rather than on a results page. Someone asks what causes sciatica, whether an adjustment will make a bulging disc worse, how long neck pain should take to settle, and gets a full answer with a handful of cited sources. The practice whose condition page was one of those sources is the practice named when the next question is who to see nearby. Nothing in a rank tracker shows whether yours was quoted, and for a business with a few miles of catchment that is the difference between a full schedule and a quiet one. Whether your condition page was one of those sources is exactly what AI Knows Us checks, assistant by assistant.
