PPC for healthcare: one campaign per service line, the Google policies that decide what can run, and conversion tracking that never carries patient data

How hospitals, medical practices and med spas build a paid search account, which healthcare categories need Google certification, why health audiences cannot be retargeted the usual way, and what to ask a healthcare PPC agency.

PPC for healthcare works when the account is built around service lines rather than the organisation, when it stays inside Google's healthcare and personalised advertising policies, and when every conversion is measured without sending patient information to an ad platform. In practice that means one campaign per service you want to grow, such as orthopaedics, urgent care or a cosmetic treatment, with its own budget and landing page. It means checking whether any of your services need Google certification before launch. And it means counting booked appointments in a way your privacy officer has approved. Get those three right and paid search becomes a predictable source of new patients.

Start from the service line, not the brand

A hospital or group practice sells many different things to many different people. Someone searching "knee replacement surgeon" and someone searching "urgent care open now" have nothing in common except your name. Put them in one campaign and the budget drifts to whichever terms are cheapest to click, not the ones that matter to you.

So decide, before you open the account, which service lines you want more patients for, and why. Capacity is the first filter. There is no point paying for clicks to a clinic with a six week wait. Margin and payer mix are the second. Then build:

CampaignKeyword themeLanding page
BrandYour hospital or practice name, with and without the townHome or location page with the main phone number
Service line, one eachThe treatment or specialty in plain words, plus "near me" and the townThe service page, with the clinicians, locations and how to book
Urgent or walk in care"Open now", "walk in", "urgent care" plus townA page that shows current hours and wait information
Recruitment, if you run itJob titlesCareers page, kept in a separate account or campaign so it never mixes with patient data

Use negative keywords early. Health searches attract students, job seekers, people looking for free information and people looking for a condition rather than a provider. Words such as "jobs", "salary", "course", "symptoms of" and "free" are typical starting negatives, refined from your own search terms report every week.

What Google lets healthcare advertisers run

Google's healthcare and medicines policy, read on 21 October 2026, says some healthcare content "can't be advertised at all, while others can only be advertised in certain locations by advertisers who have applied and been approved". Where a certified advertiser targets allowed locations, the ad shows as "Eligible (limited)". The policy lists, among others:

  • Prescription drug services, including online pharmacies and telemedicine providers. Advertisers must apply, and rules vary by country.
  • Addiction services. In the United States, recovery oriented drug and alcohol treatment providers need LegitScript certification and Google certification.
  • Health insurance, where certification applies to most advertisers.
  • Clinical trial recruitment, which is not allowed in most locations.
  • Speculative and experimental treatments, including some cell and gene therapies.

Most hospital and practice campaigns, such as orthopaedics, primary care or imaging, fall outside these categories. Telehealth services and any campaign that names a prescription drug are the cases to check first. Build certification time into the launch plan when they apply. The claims your ads make are a separate question, covered in our guide to healthcare advertising and what you may say.

Why health audiences cannot be retargeted the usual way

Google's personalised advertising policy lists "Health" as a sensitive interest category. It says advertisers promoting products and services in sensitive interest categories "are unable to use advertiser-curated audiences". That covers your own remarketing lists and uploaded customer lists. Google's own predefined audiences remain available.

That changes the plan for many healthcare accounts. A campaign built on following visitors of a cardiology page around the web will not pass review. Spend that effort on the search campaigns, where the person has told you what they want in their own words.

Conversion tracking without patient data

This is where healthcare PPC goes wrong quietly. The HHS bulletin on online tracking technologies, read on 21 October 2026, says that where a person books an appointment through a covered clinic's website and third party tracking sends the appointment details and IP address to the tracking vendor, that vendor "is a business associate, and a BAA is required". The same page notes that on 20 June 2024 a federal court in Texas vacated part of the guidance, the part about connecting an IP address with a visit to an unauthenticated public page about health conditions, and that HHS is evaluating next steps. The order was limited to that point, so tracking on logged in pages such as a patient portal was not part of it.

A workable setup:

  • Keep ad pixels off patient portals, booking forms and symptom checkers.
  • Count calls from ads through call tracking that does not record or transcribe the conversation, or through a vendor that will sign a business associate agreement.
  • Name conversions neutrally, such as "booking complete", never with a condition or treatment in the name or the URL.
  • Report booked appointments by campaign from your own scheduling data, joined to spend in a spreadsheet, rather than passing patient records back to the ad platform.

Ask your privacy officer to sign off the tag list before launch. It is quicker than removing data after the fact. This is general information, not legal advice.

Med spa PPC

Med spas sit between healthcare and retail. Many of their treatments are cosmetic, so searches are price and comparison driven, but the treatments are often medical procedures supervised under state rules. Build one campaign per treatment, show who performs it and their qualification on the landing page, and keep before and after imagery and outcome claims to what your medical director has approved. Prescription products named in keywords or ads fall under Google's restricted drug terms policy, so check that before you bid on a product name. Our page on choosing a med spa marketing agency covers the state board questions.

What to ask a healthcare PPC agency

  • Which of our services would need Google certification, and who applies?
  • How will you track conversions, what will not be collected, and will you sign a business associate agreement?
  • How will you report booked appointments by service line, not clicks?
  • Who owns the Google Ads account? It should be yours, with the agency given access.
  • What will you refuse to run? A good agency has examples.

Be cautious of anyone who promises a number of new patients before seeing your capacity and search volume, or who plans remarketing to people who visited condition pages.

Questions about PPC for healthcare

Is PPC worth it for a hospital?

For service lines with spare capacity and clear search demand, yes. Paid search reaches people who are already looking for the treatment. For services with long waits, spend the money on capacity first.

Do healthcare providers need Google certification to advertise?

Only for certain categories, including prescription drug services such as telemedicine, addiction services in the United States, health insurance and a few others. Most hospital and practice service campaigns do not need it.

Can a medical practice use remarketing?

Google treats health as a sensitive interest category, so advertiser-curated audiences, including remarketing lists, are not available for those ads. Predefined Google audiences are still allowed.

How should healthcare PPC conversions be tracked?

Count booked appointments and qualified calls without sending patient information to the ad platform. Keep pixels off booking flows and portals unless the vendor has signed a business associate agreement and your privacy officer approves.

How is PPC for dentists different?

The policy and privacy rules are similar, but a dental account is usually one location with a few high value treatments. Our page on dental PPC covers that build.

The answer that appears above the ads

For a growing number of health searches, the first thing a patient reads is a summary written by an assistant, not an ad. When someone asks "who is the best hand surgeon near me", the answer names providers drawn from listings, directories and service pages, and no bid changes it. That is a separate thing to check from your search terms report: which hospitals and practices the assistants name for each service line you pay to promote. AI Knows Us runs those patient questions through ChatGPT, Gemini and Google's AI answers so a marketing team can see whether the services it advertises are the ones it gets named for.
